According to the Ohio BMV, the following are the only acceptable forms of identification:
1. It must contain the name and date of birth and must be verifiable.
2. Social Security Number (if applicable) should document in both primary and secondary.
3. Name on documents must match the actual name of the person, unless a change of name by court order, marriage certificate or marriage is created.
4. A certified copy of the original by the issuing authority is acceptable.
5. Certified copies of documents are certified unacceptable.
6. Failure to acceptable documentation 2 result in the rejection of the application type.
